Record Release Form for All Students
Please print two copies of the Record Release Form
. Upload one copy to the MACCESS portal, and submit a second copy to the current school. All supporting documentation must be received in order for an application to be reviewed. Parents are responsible for insuring all pertinent records are sent by stated deadlines. Most documents can be uploaded to the MACCESS portal.
All applicants are required to submit two teacher evaluation forms, one from their current English/Reading teacher and one from their current Math teacher. In the BMHS MACCESS portal, you have the ability to send a copy of the recommendation form directly to your student's teachers. Teachers can fill out the recommendation and then confidentially upload it to the portal. PDF copies of these recommendation forms can also be found below.
If you wish to submit other recommendations, you may use this form.
St Joseph's program application requires a total of four (4) recommendations. In the BMHS MACCESS portal, you have the ability to send a copy of the recommendation directly to your student's additional references. Recommendations may be uploaded by the reference to the portal.
HSPT Registration Form with Dates
Online registration for the High School Placement Test (HSPT) begins October 2nd. This test is required for all incoming freshman applicants. Please register online as soon as possible by visiting the Archdiocese of Washington
St. Joseph Program Application
The St. Joseph Program is designed to provide a positive, Catholic, college-preparatory, educational and social experience for a select group of students with learning differences. Be sure to review all program information and procedures at St. Joseph Program
All medical forms are available at Magnus Health Forms are completed online or completed by the doctor and uploaded to Magnus by August 1st.
