All BMHS current students as well as BMHS applicants interested in applying for financial aid must complete and submit a FACTS Grant & Aid Application.Starting November 1, 2009, you can apply using one of two methods.
The methods are:
- ONLINE - click on this link - FACTS Tuition Aid
- PAPER APPLICATION - Available in the BMHS main office
To complete an application, you need the following information:
- Copies of your 2008 IRS Federal Form 1040, 1040a or 1049 EZ U.S. Individual Tax Return including tax Schedules C, E, F for applicant (and co-applicant).
- Copies of all 2008 W-2 Wage and Tax Statements for both applicant and spouse.
- Copies of supporting documentation for household Social Security Income, Welfare, Child Support, food Stamps, Workers' Compensation, and Temporary Assistance for Needy Families (TANF).
- Payment of NONREFUNDABLE $25.00 application fee.
- Signature required for paper applications only.
